your company needs to submit first na close na sila. get your docs such as cert of contributions, coe and such po. If nakapagsubmit ka ng mat notification sa HR niyo before pa magsara yung company niyo, hingiin mo yung mat notification files (kung may nasubmit ka na mat-1 and attachments) then ask for cert of non-cash advancement and a copy of L-501. Pero kung nabuntis ka after magsara and non-operational na yung company niyo and wala ka pang nasusubmit na mat-1, you may submit it online. You just need a certificate of non-cash advancement and copy of L-501 sa employer mo para makapag reimburse ka ng personal.
