Father's name

Kung hindi naman po gagamitin un surname ng father sa birth cert po ni baby, need pa din po bang ilagay un name ng father or kahit leave blank nalang po? Thanks sa sasagot.

Ang alam ko..and i am not sure with the process pero it should be blank.

Yes sis.blank lang dapat..kase if lalagyan nia ng tatay kailangan pirmahan nia yung sa likod na parang admission of paternity ata yun.
