Actual weeks of pregnancy

I know we suppose to count from the first day from last period, but when we count personally, do we deduct the weeks that we know we didn't xoxo, so the baby growth will be more accurate? Because if I following this app, my baby should be bigger than my ultrasound picture, so I'm little confused, thanks for answering
