Hi. Ask ko lang po if meron bang fee kapag mag change ng surname and status sa PhilHealth ID?

wala po, just bring a photocopy and original copy of the marriage certificate, fill out PMRF ma download mo cya or makuha mo sa PhilHealth office, plus ID nlang din po. pag mag add ons ka dependent, dahil mo copy and original birth certificate po

Nope, wala po, bring the necessary documents needed, pila lang po kau with the filled out form and proceed sa nearest philhealth office
