I am employed before and now voluntary paying nalang, i have a online account, need lang kapag ipa file mo sa online maternity mo is nakapagpa ultrasound kna para alam mo ang EDD mo.. then nagpunta ko sa branch and show them ung proof ng filing ko online then ok ndaw un,balik nalang ako for after manganak to file for Mat2..

Pwede niyo po download and maternity notification sa website Ng sss or pwede rin if ok lng sainyo pumunta Ng mismong branch Ng sss hingi ka po don Ng mat1 or maternity notification submit mo sa hr niyo w/ 2 valid point ID's and original ultrasound. Photocopy mo nlng Yung ultrasound mo pra may hawak ka pdn na copy.

If not employed, direcho ka na sa nearest SSS branch sa inyo sis marami ka pwedeng mapagtanungan dun on how to file. Kung employed naman just ask your HR and they will be the one to assist you 😊

if employed k, hingi klng ng form sa company mo at bgay k ng 2valid ids then orig. ultrasound. if not, punta k sss prio. k nmn dun. ganon din reqs. hingi klng form mat1 kamo.
