SSS MATBEN REQUIREMENT

Hi Moms, Ask ko lang po for separated/resigned employees kailangan pa po ba magsubmit ng SPECIMEN SIGNATURE CARD, together with COE and Certification of No advancement? Or hindi na po? Thanks sa sasagot 😘
