Good day! I just want to ask a few questions regarding the requirements to be submitted for my MAT2.
When I claimed my cheque onsite, I was given a paper by a personnel from HR that has the SSS Obstetrical History. It wasn't filled out by my obgyne, instead they have provided the obsterical history in a different paper that they issued in the hospital. Is that alright? Or the SSS form should also be filled out by my doctor?
