Requirements for MAT2

Good Day! (See attached photos below for my questions) Sana po may maka tulong medyo confuse ako. My previous employer gave me already my of employment and L501. And also nag request din ako ng certificate of separation with non advance payment sa kanila kasi needed yun for MAT2 2. Consider na din ba na certificate of separation with non advance payment itong binigay nila sa akin or its just literal na COE lang. Then I noticed din na mag kaiba yung pirma dun sa L501 and sa certificate which is ang naka indicate sa requirements/instruction dun sa sss, dapat same ang pirma nung nasa certificate and L501. Do you think guys, accept kaya to ng SSS once na i file ko mga requirements ko for MAT2? Thank you for your answer. Hingi lang din po ako ng advice para may idea ako. Salamat.

ok na yung sa letter kaso diko sure kung pwde na yung sign, magkaiba kase eh tas iba din yung name nung nagpirma

Hello mag ask lang din po ako. Paano pag may certificate of separation and non cash advancement naman pero walang L501, tatanggapin po ba? Or need talaga L501?

..San nyo po kinuha ung form l501? .. resign Ka po ba

..kakapunta ko kc kahapon SSS para sa mat2, hinahanapan ako Ng ganyan .. eeh 10yrs n nman pg sa employer ko, sa Manila kc company..
