My cousin complains that her 4-month-old son gets forceful and agitated during nursing. She tries to distract him by talking to him while feeding, but sometimes she has to take him off, let him relax, and try again. She finds it very embarrassing to feed him in public. Could it be that he's too hungry, or that she's not producing enough milk? What can she do?

It could be that she is encountering some of the common breastfeedings problems such as the flow of the milk being too fast (which will frustrate her baby) or the baby did not latch on properly (baby is not getting milk) or baby is having reflex. Other possible reasons may be that her baby may be teething or having an infection. It is best to consult a lactation expert for his/her professional advice. There are plenty of ways to correct letdown and other issues. Hopefully she will find the cause and not let this affect her experience with breastfeeding outside.
