use whatever name you use sa SSS and Philhealth so you won't have problems claiming your benefits. The baby's birth certificate is independent naman, fill it up however you like. Sa Philhealth when you update your MDR to claim for your baby's benefits ilalagay mo naman fullname ng baby eh so they will credit your baby as your dependent.

if you're using your married name sa IDs and you have time I suggest update all of it to use your maiden name. Especially sa SSS and Philhealth. So that when you fill up the birth cert you can use your maiden name and use whatever surname you want for your child.

much better po to consult sss employee para sigurado.. in my opinion , kahit sino pa po ang tatay ng anak nyo at kahit nakaapelido po siya sa new partner nyo basta po may contribution ka po sa sss ay makakakuha ka po ng maternity benefit

Yes pwede mami. Mama ko din kasal sa una matagal na silang hiwalay ng papa ko. Then na buntis si mama ng current partner nya yung bunso ko namang kapatid naging surname naman ng partner ni mama. Btw 11 years old na bunso naming kapatid. US military daddy nya na hiwalay din sa unang asawa.
