19 Month Nappy Rash

Hi. How long usually does rashes go off? My LO is having bad rashes at her private. It's red and spotty, itchy coz LO keep wanting to sctrach after bath. I apply desitin and drapolene but it's been a week and doesn't seem to go off. Any other cream to recommend?
