helow po..almost more than 6yrs na kami kasal ng partner ko kaso hindi po naasikaso yung marriage contract namen nung time ng church wedding namin kasi nagkaproblema, sadly until now wala pa rin at magkakababy na kami..pwede ko po ba gamitin surname ng hubby ko sa birthcert ng baby namin kahit walang marriage contract? salamat po sa sasagot...

yes, if married namab talaga kayo, just put there both your status as married and they won't ask naman, indicate the father sa form and have him sign.. don't stress.. sa hospital pa lang you fill up the form and they will take care of the rest..
