Good Evening Dr, I would like to ask about cervical cerclage. Arabin pessary McDonald cerclage shirodkar cerclage why don't government hospitals offer shirodkar cerclage?as far as I know that is the best method compared to arabin pessary. Why doctors at government hospital, do not focus on cases like this? why they have to wait for several miscarriages in the second trimester to declare that the mother has a weak cervix? even though the cause of the miscarriage is not from any bacterial infection. there are mothers who have been confirmed to have the problem and need to get cervical cerclage for the next pregnancy but when the time comes the doctor denies and delays until she miscarries again.

Thank you for asking a very relevant and an important question. Not all second trimester pregnancy losses are associated with a cervical insufficiency and hence it is very important to see the right expert for a proper evaluation and diagnosis as the causes are multifactorial. If one is truly confirmed to have cervical insufficiency, my advice is for you to see the right expert. Vaginal pessaries such as Arabin are not recommended as it is associated with a higher risk of infections. There are many different types of cervical cerclage and Shirodkar cerclage, or a high vaginal cerclage is not proven to be superior to a low vaginal cerclage. If you see the right expert, than there would be able to advise you on the best treatment and interventions that you need. Not everyone needs a Shirodkar cerclage. It is important to see the right expert for a proper diagnosis and advice.
