If employed ka employer nag aasikaso nian. Go to your HR. Dalhin mo lahat ng proof na preggy like like ultrasounds at lab test para masubmit na nila ung maternity notification.

If employed, submit Mat1 notif with ur ultrasound results sa HR mo. If unemployed, you can submit it online. After delivery, submit Mat2 and certificate of live birth.

if employed po kayo si HR na po mag aasikaso. need nio lang po inotify si hr po dala kayo ng med cert or ultrasound result :)
