Hi Rosielyn. I want to clarify more about your baby's relation to your husband just so I can properly address your concern. I hope you don't mind. Whether or not your baby is a biological child of your husband, prior to submission of birth certificate, you must provide a proof of filiation. Then, what your husband should do is sign an affidavit saying that he acknowledges your child. But if your baby is not a biological child of your husband, I believe you should jointly file an adoption for your child in order for him to carry your husband's surname. For further clarifications you can go ask a lawyer just to be sure.
