Are there additional requirements if we want to follow the name of my baby boy to my husband's name?

Hi Rosielyn. I want to clarify more about your baby's relation to your husband just so I can properly address your concern. I hope you don't mind. Whether or not your baby is a biological child of your husband, prior to submission of birth certificate, you must provide a proof of filiation. Then, what your husband should do is sign an affidavit saying that he acknowledges your child. But if your baby is not a biological child of your husband, I believe you should jointly file an adoption for your child in order for him to carry your husband's surname. For further clarifications you can go ask a lawyer just to be sure.

No. I don't think so. It's just the same, you just have an additional suffix for it. Just make sure that all the details are correct so as to avoid the hassle of correcting them.
