TIPS ON HOW TO AVAIL SOCIAL SERVICE CARD FROM ST LUKE'S MED CENTER

SHARING IS CARING ❤️ Hi mga momsh! Gusto nyo bang manganak or magpa alaga sa mga magagaling na OB ng St. Luke's but cannot afford its fees or gusto nyo lang talagang magtipid ng pera but still wants to experience the private care of st. Luke's? I will list down the requirements and benefits of availing SS card from St. Lukes. Step by step guide on requirements : 1. A pregnant woman should not be higher that 3mos pregnant upon filing an application coz they prefer to monitor your pregnancy from 1st trimester until the end of your pregnancy journey. 2. Bring the following: transV ultrasound, valid id and one adult companion. Hindi po pwedeng walang kasamang adult at yung kasama nyo po ay kailangan ding magdala ng ID, as much as possible, sya palagi dapat ang kasama nyo during scheduled check-ups. Also avoid bringing more than 1 companion. 3. Go to St. Luke's Social Service office on or before 7am (mondays to Fridays), ask the guards po kung saan ang office nila. 4. Social service will conduct an initial interview to you and your companion (such as address, age, work, etc) 5. Bibigyan din nila kayo ng list ng iba pang requirements such as lab tests and general requirements like payslip and an application form. 6. Then they will proceed in giving you a schedule for your 2nd visit (2-5 days from your first visit) para mabigyan ka ng time to prepare for the requirements. DAPAT KUMPLETO NA ANG REQUIREMENTS ON THE 2ND VISIT. 7. Your 2nd visit includes, meeting and initial check up with the OB, filing of requirements, and 2nd interview with the SS officer (dito mo na malalaman kung anong Class ng card ang ibibigay sayo. Mine is class B, free check up, 75% off on laboratories and 60% off on hospital bills. Class A is 50% Off, class C is 80% off and class D is 90% off. Your Class will be determined by the ss officer based on your interview coz they find out kung ano ang capacity to pay mo. 8. They will require you to attend mama's class, a 2 hr seminar with the OB and pedia. And the social service seminar, 2hrs seminar about sa mga benefit na makukuha mo and the do's and dont's. 9. Pag nakumpleto mo na ang mga requirement, they will give you your card, but if the card is not yet ready, they will give you a temporary card para magamit mo na sa susunod mong check up ang benefits nito. Benefits of ss card: 1. Generous discount based on your Card Classification as mentioned above 2. The ss card of a pregnant woman will expire a month after mong manganak but they will automatically give your new born child an ss card for the pedia consultation, medication and vaccines na kailangan ni baby at 3 yrs itong valid. Free check ups with baby's laboratory, vaccines and confinement discounts. 3. You can always renew your own card in case of another pregnancy, same procedure. 5. Kung meron kayong Pap smear result, you may also bring it on the 2nd visit kasi kung wala, you will have to pay 700.00 for pap smear dahil wala pa kayong ss card at that moment. 3. Mas mabuti pang umabsent kaysa malate, pwede ka namang magpa re-sched on the next week. 4. They might require you to go to St lukes college of medicine (sa likod lang ng hospital) ipapa interview ka sa mga estudyante nila (future OBgynes of st. Lukes) this is part of their learning progress. They will not conduct lab tests on you, simply interviews and kakapain lang ang tyan mo, they were also accomodated by their professor who is also an experienced OB. And lastly, they were all nice. 5. THEY CAN DECLINE AN APPLICATION. They also conduct surprise visits sa address na binigay nyo so dont ever fake your requirements. Hope this helps! Have a great hump day mga momshies!

Not sure kung kelan sila magreresume. But in your case, yes po pwede kayong kumuha ng ss card once na magresume sila.

Wow may ganito pala ang St. Lukes.. Kailan kaya sila magreresume?
