Maternity Leave po? If you're referring to maternity notification just fill up the form MAT1 tas attach mo ultrasound result. Then after mo manganak fillup ka MAT2 tapos attach mo yung MAT1 mo na may tatak ni SSS na received, Birthcert ni baby, if CS kailangan may OR records ka po. Pero kung maternity leave po ang tanong mo talaga, I think it's best to ask your HR officer about that. ☺
