Maternity Leave po? If you're referring to maternity notification just fill up the form MAT1 tas attach mo ultrasound result. Then after mo manganak fillup ka MAT2 tapos attach mo yung MAT1 mo na may tatak ni SSS na received, Birthcert ni baby, if CS kailangan may OR records ka po. Pero kung maternity leave po ang tanong mo talaga, I think it's best to ask your HR officer about that. ☺

If maternity notification , utz w/ pic na original and mat 1 form Mat 2= certified tc ng birth cert 2 pcs, ob history, umid card and 1 company id(photocopy lang den lagyan mo ng signature 3 , static info, if Cs kasama ang operating room record

Hingi ka ng form sa sss ng mat1 sabihin Mo mag file ka ng mat1 tas dala ka ultrasound Ni baby kung buntis kapa basta may hulog ka na updated ahh

Kung voluntary po kayo need nyo magfile ng mat1 sa sss mismo pero kung may employer kayo hr nyo po magasikaso nun. Notify nyo lang sila.
