ang hulog ko lang po ay august 2019 to december 2019 na stop po kasi lumipat ako ng work. feb 2020 na po yung sunod na hulog hindi na po nasundan. sa july po ang labas ni baby. mahahabol ko pa po ba kung huhulugan ko yung mga months na walang hulog? or hindi na po 3 months preggy po ako ngayon thank you

Mat 1,Mat2 form, SSS Id kung wala 2 valid ID, CTC ng Birth cert. ni baby, (kung CS Operating room record, medical abstract, delivery report kahit isa or mas maganda lahat meron), kapag may bank account na updated bank slip kung wala need magopen pa. Kung kakaresigned lng sa trabaho (L501, non-cash advancement and cert. of separation)
