SSS Maternity Benefit for Self Employed

Good am po. Nagko-contribute po ako sa SSS as self employed. Ano po ba need ko gawin or ipasa sa SSS para makapagclaim ng maternity benefit? Im 28 weeks pregnant po. Thanks po.

Ultrasound and valid id's momsh. Then pag andon n po qau may papa fill up po na form. Mat notification ☺
