Go to your LGU vaccination websites. Like: For QC: https://qceservices.quezoncity.gov.ph/qcvaxeasy For Makati: http://mmc.link/A3PediatricCEIR (Form from Makati Med where my kids and my sister-in-law pedia is)           https://www.covid19vaccine.safemakati.com/ Wait for the confirmation and schedule that will be sent via SMS and Email that you registered.  Ask your child’s Pediatrician for medical clearance or certificate if your child is good to take the vaccine. Prepare the following: Birth Certificate of the Child. Photocopy it Valid of the Child, like school I.D. Photocopy it Valid I.D of the parent who will accompany the child. Photocopy it If the child’s parents are not available. Guardians or representatives of the parents to accompany the child. Providing a Letter of Authorization from the parent/s.  If the parent/s of the child is already dead, provide the dead certificate of the child’s parent/s. Proof of guardianship over the child or relationship with the child. Like; Guardian’s Birth Certificate.  If the guardian of the child is not available on the plotted date of vaccination, Provide a Letter of Authorization for the representative with an attached photocopy of both guardian and representative’s ID.  Print the documents and forms from the LGU sites.  ( in the case of Makati Med. Patient's Information form, and Screening Questionnaires, Covid-19 Immunization Waiver Form,  Makati Medical Center Covid-19 Vaccination Screening form, Pfizer Pediatric Vaccination Consent form and assessment, and Pedia Health Declaration form.   • If the child had RT PCR bring the result with the date and result. If the child was tested as positive must provide a clearance that the child had completed the Quarantine period and the event was over the past 2 months ago. • The Child must not receive any vaccination for 2 weeks or a month. #TeamBakuNanay #VaccinationWorksForAll
