Copy of Bank Account (with printed name and account number) 4. 2 valid IDs (preferably government issued) 5. Sa akin derect na ako nagpasa ng mat2 thru online kc di ako nakapasa ng mat1... Nagpasa ako mat2 requirements ko birth certificate ni baby sana affidavit of undertaking tapos nagpasa register ako ng disbursement na thru M LLUILLER ko kuhanin ang pera Pra di na ako kukuha ng ATM.. Away ng dios nag file ako August 3 approved na augost 9 tapos nakuha kona ang pera augost 11 thru M LLUILLER.. ang bilis lang pag sa online ka mag pasa ng maternity 😊❤

magtatanong lang din po ako paano po kung through online po ako nag submit ng MAT1

yung hospital records nyo po. need yan ni sss.

Savings account po tas yung mat 1 na naifile mo nun..

How about cesarean delivery po?
