Mat 1 or maternity notification, magsasubmit ka sa sss ng ultrasound mo notifying them that you are pregnant and informing them na gagamit ka ng SSS maternity benefit (if qualified). MAT 2, magsasubmit ka ng mat 2 forms kasama ng ibang supporting docs (birth cert) para matanggap mo yung benefit.
