sis, pwede ka na magupdate sa online sa SSS pati sa philhealth basta meron kana po marriage certificate..
for SSS:
1. login ka sa SSS portal
2. Click mo E-Services >> Request Member Data Change (Simple Correction)
3. Check the Civil Status at ilagay po ung mga hinihingi na details
4. Upload nyo po ung marriage certificate ninyo and wait nyo lang po mga ilang weeks for approval.
for Philhealth:
1. Download PMRF Form and fill out nyo lang po ang details pero check nyo po ang Updating/Amendment sa upper portion ng form.
2. Sa 2nd page check nyo din po kung ano ung iupdate nyo like Change/Correction of Name and Civil Status and sign nyo po ang form.
3. Attach nyo po sa email ang filled-out form pati po marriage certificate and send to [email protected]
wait nyo na lang po ang email confirmation with your updated MDR
