Kailangan po mag submit ng Maternity notification sa SSS. They will provide the form na need to fill up. Bring photocopy of 1st ultrasound and UMID id. If wala po umid, photocopy ng 2 valid ids.

Dapat po member ka ng SSS tapos saka ka po magfile ng mat1.. dapat may hulog ka dn po para maging qualify ka sa mat benefits.. pwede ka mag voluntary po

u have to be a member with active contribution, then punta po kayo office nila with ultrasound fill up ng maternity notification (mat1).
