kung available naman ang birth cert dalhin na lang din para sigurado. 😊 eto mga dinala ko (sigurista hahaha) marriage cert birth certificate philhealth (galing na sa office ni hubby) valid ids all with photocopies 😁 at sa small index card sinulat ko lahat ng details ko and hubs para sa mga fifill up na forms
