Hi mga mi, 36weeks na ko and nakahanda na lahat ng need dalhin sa hospital if ever manganganak na ko ang di na lang naka handa is mga documents, ano po ba mga documents needed dalhin sa hospital. Ps. Private hospital po ako. Thank you.
phil health ID, Marriage certificate, birth certificate niyo ng husband, maternity book, vaccination card, copy ng mga diagnostic test. sana nakapag maternity notif ka na po sa sss and updated ang contribution
Additionally po if employed po kayo mi secure cf1 and csf form kay employer. Need po sa private hospital for philhealth usage.
resign na po ako last july, magagamit ko pa din kaya ang philhealth ko? di ko na po sya nahulugan till now.
Eto po ang nasa website Payment of at least 3 months’ worth of premiums within the immediate 6 months of confinement. For pregnancies, availing of the newborn care package, dialysis, chemotherapy, radiotherapy and selected surgical procedures, 9 months’ worth of contributions in the last 12 months is needed.
