Documents needed

i brought valid IDs, MDR ng philhealth. nag-fill-up ako ng form for philhealth sa hospital pero hindi na hinanap ang MDR. dahil kita nila sa online ang details ko. but still bring it, just incase.

Lab test results, mommy booklet, nso birth certificate and id (mommy and daddy) , philhealth form, ultrasound results

Hi mommy, Valid ID and copy of MDR (philhealth) lang naman lagi ko dala ☺️
