Hinde ko alam kung ano ipapagamit ko kasi matagal kona ginagamit last name ng papa ko. Pero sa bc ko nakalagay sa mama ko. Wala ako medlle name. Ano po kaya ang pwd ipagamit ko sa bby ko. Yung nakalagay sa bc. Or yung matagal kona ginagamit na last name ng papa ko. Patulong naman po.

I assume you are born out of wedlock since wala kang middle name. Under the law, illegitimate child MUST use the surname of the mother. The only way for you to be able to use the surname of your father is when you are recognized at the back of your birth certificate; which is not the fact in your case. Hindi ko alam bakit mo ginamit ang surname ng father mo. I am sure magkakaproblema ka sa mga papers mo later because of that discrepancy. Para hindi na magkaproblema ang anak mo (which i assume is born out of wedlock din since you did not mention anything about the child's father) I would suggest na cause the child to use your "law mandated surname:, the one reflected in your BIRTH CERTIFICATE.
