Question

Hi, my child is currently having eczema thus I had requested Sparkletots infant care to put moisturiser for him aft bath. Can i check if it is required to fill up a form everyday just for them to put a normal over the counter moisturiser (not doctors prescription)? Before enrolment there was a things to bring list and moisturiser was included in it.

hi. dont have to fill up form everyday. for my side we only have this non-prescribe form for all the creams. teachers will fill in the details and parents have to sign every month. this to ensure that we did our part to apply all the creams that parents wants.
