Ask ko lang po sino na po nagpasa ng matben nila sa sss ask ko sana about sa birthcertificate pwde po ba ipasa kahit na certified true copy yung birth certificate or required din na dpat psa na yung birthcertificate thanks sa sasagot
Nakalagay naman po sa likod ng mat2 form ang required documents. Alam ko po cert true copy ang nirerequest. Pero kung may PSA na pwede din naman. Basta need lang ng proof na nairegister na si baby mo
yung sakin sis ganon ☺ pinakita ko siya sa sss sabi pa register ko daw then pa certified true copy☺
Pa register mo lang sa munisipyo nyo, basta may tatak at registry number, yun naman importante 🙂
