Covered ang first 4 deliveries or miscarriages.
Within 60days after conception dapat nakapagsubmit ka ng Maternity Notification Form & Proof of Pregnancy (Ultrasound Report).
After ng delivery magsubmit ka naman ng 1. Maternity Reimbursement Form
2. Duly stamped na Maternity Notification Form
3. Certified true or authenticated copy of duly registered birth certificate.
Yung sa akin like about after a month dumating na yung cheque sa house namin 😉
Kapag po ba resigned na unemployed na , need pba ng maternity notification? Voluntary na kase ko naghuhulog . Sabe ko kase sa sss kung magffile pako ng notification . Sabe nya basta maghulog lang daw ako.
