As per RA 9255 : 0-6 yrs. Old revised IRR of art. 176 rule 7&8 Requirements : 1. Valid ID and cedula of Father and mother (xerox) 2. Fee is php330 3. Affidavit to use the name of the father Yan po yung nakalagay sa binigay sakin ng lying in po. Check niyo na lang sa lying in po kung ganyan din ang price. Nasa province po kasi aki baka iba ang price sa Manila.
