Dressing a 3MO

Hi.. what do you all dress your 3 month old babies in especially those with sensitive skin and have frequent skin rashes? Thanks in advance. Edit: do you all still help your babies put on a bottom such as long pants? Is it too hot for baby to wear a diaper, romper and then pants?
