im employed. regarding sss mat ben, i just went to our HR. HRD gave the mat 1 form. i submitted the filled-up form and requirements sa HR. sila ang nagprocess. before manganak, binigay ang mat benefit sakin. mat 2 form and requirements naman ay after manganak. again, sinubmit sa HR for them to process. kindly discuss it with your HR.
