sickness leave

Kelangan ma notify mo yung employer mo within 5 days after mag start ng sickness or injury. Bibigyan ka nila ng Sickness Notification Form, fill out mo lang yun tapos may part dun sa form na kelangan i fill out at pirmahan ng attending physician mo. Tapos within 5 days after mareceive ng employer yung notification mo, kelangan ma notify na nila si SSS.
