Birth certificate

Hi mga momsh! May itatanong lang po ako kung possible pa ba na pwede palitan or baguhin yung birth certificate ni baby? Like for example aalisin yung lastname ng father nya? Tapos yung gagamitin na lastname yung sa mother? Pero di po kasal. Thank you
