Yes po. No need na yung thumbprint if able to sign naman na kayo. Hiningan din ako ng HR namin ng summary of contributions sa SSS together with 2 valid IDs. If employed po kayo, be prepared nalang din po to provide such documents for faster transaction.
a. ) kung pang ilang anak mo na at kelan expected due date mo b.) kung pang ilang panganganak mo na , kung pang ilan kunan kana 0 kung wala. c.) Buong Pangalan , pirma , date dun kang wala kang lalagay sa witness kapag nakakapirma at sulat ka naman.

if employed po kau, submit nio po sa employer nio. then they will process it with the rest of your requirements. i think 5 ung forms na need.

Yes fill up mo lang ung delivery date mo and if 1st or 2nd pregnancy and such. pirmahan and lagyan ng thumbprint.

mas madali po kung sa sss portal kayo mag apply para di na kyo pumunta ng sss branch

just fill in A, B, C mommy.. no need for the fingerprint and witnesses if you can sign naman. Mat 1 po ba yan, HR na po pala sa other info just attach ultrasound upon submission for the notification processing
