Marriage contract copy. Monthly checkup from OB. (kung yung ob mo is from private or from other hospital) Copy of Philhealth MDR

Kailangan papo kaya mag dala ng mga records ng monthly check up kahit ung ob ko po ay mismong sa hospital na pag aanakan ko?

Marriage Contract Valid ID's Philhealth MDR Philhealth Contribution Record (from employer if employed) Ob'gyne's referral letter
