Yes sis HR dapat magprovide ng forms with company's details na yun. Always follow up. Make sure may mareceive ka confirmation receipt kapag nagsubmit ka na requirements sakanila and then follow up lang. From my experience marereceive ang mat benefits 1 month before Expected due date. Hinintay ko talaga makuha muna yun bago ako magLOA. I even asked for sample computation and estimated date kailan ko makukuha. Thank god i got them in full. Tip is always communicate sa HR either via email or in person 😊
