Kahit po certificate of live birth lang muna momsh, as long as certified true copy ng city hall. Pwede din po photocopy lang ang ipasa basta po certified true copy na.

Hi mamsh. Ok lang po ba yung green na form na certificate of live birth then pa CTC lang po sa civil registrar?
