I think shouldnt be an issue! I changed my pd from private to government (polyclinic). My baby initally had Jaundice since birth so we continue with private pd since I chose to gave birth at TMC.
Now that he is in pink health, we changed to polyclinic, definitely more waiting time but routine checkup and vaccination, polyclinic is good enough. For new pd, you can show them baby health booklet, if there's anything else you feel you should update the new pd, please feel free. If not, usually they will just see the health booklet and ask a few simple questions like how much is the milk intake & frequency etc
