Philhealth self employed/voluntary need ng affidavit of declared income?

Hi! Ask ko lang po kung need po ba talaga yung affidavit of declared income sa philhealth? Nag update ako ng philhealth from employed to self employed/voluntary pero wala akong nilagay na income so nagkaproblem po ako sa pagbabayad thru gcash. Kaya inutusan ko yung sister ko na iupdate and magbayad sa philhealth pero hinihingan sya ng affidavit of declared income. Ano po pwede gawin?

Sundin nyo na lang po yung requirement. Gumawa na lang po kayo, icheck nyo online if may available form/ format. Pwede rin kayo magpatulong sa PAO, diretso ipa-notaryo nyo na at isubmit sa philhealth. Ako naman before, since naging unemployed at naging inactive na yung philhealth ko, hinulugan ko ulit para magamit sa panganganak, pero hiningan ng Affidavit of No Income for the period na hindi ako naghulog. Nagsubmit ako kaya um-ok naman at nahulugan ko na after.
