depende po kung anong type ng Philhealth nyo, kung indigent po at ma-aadmit po kayo sa isang Philhealth accredited at public na hospital, may medical Social Worker po Tayo/Philhealth rep na mag-aassess po sa inyo kung qualified po kayo under POS, ito po Yung Philhealth na grina-grant ng mismong hospital para makagamit kayo ng Philhealth, need po ng valid id (if single)/ marriage contract (if married)🤗
halimbawa po, e "practicing profession" po kayo or assessed as capable ng isang social worker, need po magbayad, 400 po monthly contribution ngayon mami🤗
