What am I Thankful for in 2021.
What am I thankful for in 2021? Despite of these uncertanties there are still a lot of things I am thankful for in 2021: first is my Family's health, that everyone is in good health, food in table, continous income and being able to be together despite of the situations and lastly my baby girl and my sister's surviving Covid19, no words can express how grateful I am for all the love and support I have received during those trying times. We may have a lot of why's and hard times at the moment, but eventually this too shall pass. Just keep the faith and keep on praying. Stay safe and God bless y'all! #TAPpyHolidays2021

SSS (MAT2) MATERNITY BENIFIT REQUIRMENTS (applied online)
SSS Maternity Benifits Online Requirements: Good day Mommies! Last July few days after my Baby turns 1 year old, my Maternity Benifits was approved and it was done only Online! I've submitted the forms and all necesssry requirements online, so I've decided to make a post about the Requirements: (I was a voluntary member) 1. Maternity notification duly stamped received by the SSS prior to the date of childbirth/miscarriage 2. The member must have paid at least three monthly contributions within the 12 month period immediately preceding the semester of childbirth or miscarriage. 3. a. For Normal Delivery • Certified true copy or authenticated copy of Birth Certificate duly registered with the Local Civil Registrar. b. For Cesarean Delivery • Certified true copy or authenticated copy of Birth Certificate duly registered with the Local Civil Registrar. • Operating Room Record or Surgical Memorandum duly certified by the hospital where the member is confined. c. For Stillbirth • Fetal Death Certificate duly registered with the Local Civil Registrar. d. For Miscarriage or Abortion • Pregnancy test before and after miscarriage/abortion. • Medical Certificate/Obstetrical history indicating the number of miscarriages duly certified by the attending physician with his license number, printed name and signature; or • D & C Report duly certified by the authorized hospital representative where the member was confined. 4. a. For Separated Member • Certification from last employer with the effective date of separation from employment. b. For Voluntary Member • a copy of approved SS Form E-5 c. For Self-employed Member • a copy of approved SS Form RS-1 My MAT2 Application was done online and within 2 days it was approved, my benifits was sent to my GCASH account and was credited after 1 week.
