Hi po. Ask ko lang if need paba ng affidavit to use the surname of the father pag mag papareg. ng birth certificate ni baby? And ano po requirements sa pag papareg.

cedula nyo po parehas kung d kau kasal.. may fill upan sa likod ng bcertificate.. affidavit po un to use the surname of the father
