Birth Certificate requirements sa ospital

Hi po! Ano po requirements for birth cert? Kelangan po ba ng marriage cert or pirma Lang namin ng asawa ko? Gusto ko kasi maayos agad sss benefit ko at Ayoko na pabalik balik sa ospital. Thanks!
