If your kid's still an infant, you won't need an appointment. You can just go straight to DFA with complete requirements. You'll find the list of requirements here: https://www.passport.gov.ph/requirements/new-application. Prepare original documents and photocopies. If your child is over 1 year old, then you'll need to schedule an appointment, which you can do online here: https://www.passport.gov.ph/appointment. Then just choose a DFA office (Aseana, Megamall, Ali Mall) and an open slot in their calendar. You'll get additional instructions via email. Basta make sure you show up early, at least 30 mins early.
