1. If due date is Jan. to Mar. 2021: You should have paid at least 3 monthly contributions from Oct. 2019 to Sep. 2020 2. If due date is Apr. to June 2021: You should have paid at least 3 monthly contributions from Jan. 2020 to Dec. 2020 3. If due date is July to Sept. 2021: You should have paid at least 3 monthly contributions from Apr. 2020 to Mar. 2021 4. If due date is Oct. to Dec 2021: You should have paid at least 3 monthly contributions from July 2020 to June 2021
